In 2011, the Illinois General Assembly passed legislation mandating that 50 percent of Medicaid recipients be in a “Care Coordination Program” by 2015. Since then, the Illinois Department of Health Care and Family Services (HFS) has been working on implementation of this law through the Care Coordination Innovations Project.


Attend this free webinar to learn about care coordination, including the Care Coordination Innovations Project, the impact of these projects on health, housing, and human service providers, and what impact these projects will have on the Illinois Medicaid program’s long-term fiscal health. Hear from panelists representing a wide range of provider sectors, including: hospital, pharmacy, primary and behavioral health care, housing and human services. Panelists will also be joined by Senator Heather Steans who will share her thoughts on care coordination as it relates to the Illinois Medicaid program’s fiscal health.
