|In 2011, the Illinois General Assembly passed legislation mandating that 50 percent of Medicaid recipients be in a “Care Coordination Program” by 2015. Since then, the Illinois Department of Health Care and Family Services (HFS) has been working on implementation of this law through the Care Coordination Innovations Project.
